AND STATEMENT OF CONFORMITY

I. PERFORMANCE
|
|
| |
A. Products are tested to specific conditions to
determine results. For example, a product may be tested to a certain force,
pressure, or temperature (see categories list). The cost for these type of
test will include set-up plus per test cost. Standard test results are
provided. A one-time certification may be provided pursuant to a Hold
Harmless Agreement. B. Products may be
tested to establish performance or failure. The results of the test will
include data and recommendations as to operating and failure conditions. The
certification and cost is the same as stipulated in I-A above.
C. Products may be tested to meet certain
industry or government standards. The standard largely dictates the cost. We
have thousands of standards on file. However, purchases of new standard(s)
and assessment of the testing requirements will be charged at a fixed
minimum prepaid. |

II.
CERTIFICATION LISTING TEST
|
|
|
A. The product may undergo testing the same as I-C above.
However, in addition to certification, the product is "Listed". The listing
requires meeting certain standards. review of all drawings, initial
inspection at the manufacturer's facility and annual re-inspection. The
product is issued a listing certification, number, and on-product label (see
below). Our label is registered with the U.S. Department of Commerce. Annual
renewal testing is charged at an hourly rate, plus expenses, and a fixed
renewal fee.

PRODUCT TESTING GENERAL PROCEDURES

Product and/or materials testing at System Engineering
And Laboratories is conducted following the general procedures set forth as
follows: |

Phase I: Pre-test Research |
|
|

A. A sample of the
product or information about the product is shipped to S.E.A.L. for
familiarization.
B. Standards are
researched for applications to the product and for testing assessment.
C. A test proposal is
developed and submitted for approval.
D.
The charge for
Phase I is $450.00 payable prior to commencement of Phase I, Item B. |

Phase II: Test Approval |
|

|
A. A
contract (General Terms and Conditions) is required prior to testing. S.E.A.L.
Requires a "Hold Harmless" and "Indemnification Agreement" for any for any product tested (part of
contract, Section III, A).
B.
Client accepts test proposal, cost, and scope of work.
C.
Customarily, fifty percent (50%) of the test cost is required prior to the
start of work with the remainder due prior to S.E.A.L.'s final report and/or
product certification. |

Testing may be of a continued year to year re-certification or listing.

STATEMENT OF TESTING
CONFORMITY |
|
| |
Engineering
research, analysis, studies, and reports are governed by certain standards
and requirements. The purpose of this statement is to confirm that these
standards and requirements are followed.
System Engineering And Laboratories (S.E.A.L.) is
an independent third party Consulting and Products Testing / Research
Laboratory. S.E.A.L. is a recognized member of the American Council of
Independent Laboratories (ACIL) and many other applicable organizations.
S.E.A.L. does not subscribe to outside review services.
Additionally, for your protection and as your
subcontractor, S.E.A.L. maintains Worker's Compensation as well as a full line
of professional insurance coverage.
S.E.A.L. conforms to ANSI standards, ISO 9000
principles (International Quality Control Standards), and the International
ANSI / ISO / IEC 17025 Standard, "General Requirements for the Competence of
Testing and Calibration Laboratories". |
